Protected health information created in the course of dental treatment is governed separately by HIPAA and our Notice of Privacy Practices, available on request.

- Patient information: date of birth, dental and medical history, insurance details, and payment information, collected when you become a patient.
- Automatically collected: IP address, browser and device type, pages viewed, and dates and times of your visit, through cookies and analytics.
- Communications records: calls, texts, and emails between you and our practice, including your opt-in or opt-out status for text messaging.

- Schedule, confirm, and remind you about appointments
- Respond to your questions and requests for care
- Coordinate visits with facilities, caregivers, and family members involved in your care
- Send text messages you have consented to receive

3. Who we share it with

SMS consent is not shared with third parties. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. If you provide your mobile number and consent, we may send appointment reminders, scheduling updates, care instructions, billing notices, and replies to your messages. Message frequency varies. 5. Your choices
Reply STOP to any text to opt out, or call or email us and we will remove your number. You may request access to or correction of your dental records as provided under HIPAA and Nevada law. Nevada residents may submit a verified request that we not sell covered information under NRS Chapter 603A; we do not sell personal information.
